About the Job

The Senior Physiotherapist provides basic to complex cases of physiotherapy services to residents of the Adult Disability Home and clients of the Day Activity Centre, with a focus on optimising mobility, physical function, safety, participation, independence, and quality of life.

The role provides clinical leadership within Physiotherapy and works closely with the Head of Therapy and Multidisciplinary Team (MDT) to develop, implement, and continuously improve therapy services for adults with physical, intellectual and associated disabilities.

Clinical Practice and Service Delivery

1. Provide comprehensive physiotherapy assessment, intervention, review, and evaluation and complete necessary documentation as required for ADH residents and DAC clients.
2. Assess mobility, gait, balance, strength, endurance, transfers, posture, physical function, pain, and other relevant physiotherapy needs.
3. Formulate individualised treatment goals and intervention programmes in collaboration with beneficiaries, caregivers, support staff, and the MDT and complete necessary documentation as required.
4. Assess, formulate, implement, monitor, and evaluate physiotherapy programmes aimed at facilitating, restoring, improving, and/or maintaining functional abilities with an integration focus on their social activities’ engagement and participation in adaptive sports and other recreational activities for ADH residents and DAC clients.
5. Provide appropriate seating and positioning assessment and recommendation followed by vendor liaison and fitting sessions for ADH residents and DAC clients. Ensure proper clinical assessments and documentation completion on time.
6. Develop and implement physiotherapy programmes encompassing rehabilitation, maintenance, wellness, mobility, physical activity, and falls prevention.
7. Review beneficiaries’ progress regularly and modify treatment goals and intervention programmes according to changing needs.

Care Planning and Multidisciplinary Practice

1. Participate in Individual Care Plan (ICP) reviews, case conferences, admission and review processes, and other relevant collaborative and MDT meetings and complete necessary documentation as required.
2. Provide professional recommendations and updates regarding beneficiaries’ mobility, functional status, progress, risks, and therapy needs.
3. Collaborate closely with Occupational Therapists, Speech Therapists, Therapy Aides, Care staff, Social Service staff, and other professionals to provide holistic and coordinated care.
4. Coordinate with relevant staff on physical activity, social, recreational, and wellness programmes that promote meaningful participation and community inclusion.

Mobility, Equipment, and Safety

1. Evaluate and recommend appropriate mobility aids, positioning supports, transfer equipment, orthoses, and other rehabilitation devices to optimise function and safety.
2. Provide guidance to staff and caregivers on safe mobility, transfers, positioning, and use of prescribed equipment under seating and positioning category.
3. Promote falls prevention and safe mobility practices across the ADH and DAC.
4. Maintain and coordinate the inventory, acquisition, maintenance, and repair of physiotherapy and rehabilitation equipment.
5. Ensure equipment is appropriately used, maintained, and reviewed according to beneficiaries’ changing needs.

Clinical Leadership, Supervision, and Staff Development

1. Provide clinical guidance, mentoring, and support to junior therapists, Therapy Aides, Care staff, volunteers, and other relevant personnel within the scope of Physiotherapy.
2. Support the Head of Therapy in developing staff competencies, workflows and maintaining appropriate standards of clinical practice.
3. Conduct or contribute to training and competency development in areas such as mobility, transfers, positioning, falls prevention, exercise, and safe handling.
4. Promote evidence-based, person-centred and holistic care Philosophy Physiotherapy practice across the ADH and DAC.

Quality Improvement and Service Development

1. Work with the Head of Therapy and Rehabilitation team to review workflows, identify service gaps, and recommend improvements.
2. Contribute to the development, implementation, and review of therapy service standards, outcome measures, and quality improvement initiatives.
3. Support audits, data collection, outcome monitoring, and service reporting where required.
4. Ensure timely and accurate clinical documentation, progress monitoring, reassessment, and goal adjustments for all beneficiaries.
5. Contribute to the continuous development of the Therapy Model of Care for the ADH and DAC.

Other Responsibilities

1. Work independently in clinical assessment and decision-making while escalating complex clinical or operational matters appropriately to the Head of Therapy.
2. Maintain professional standards and comply with relevant organisational policies, clinical guidelines, and regulatory requirements.
3. Perform any other relevant duties as assigned by the Head of Therapy or Management.

Requirements

• Possess a recognised Degree / Diploma in Physiotherapy.
• Full registration with the Allied Health Professions Council (AHPC) and eligibility to practise as a Physiotherapist in Singapore.
• At least five years of clinical experience.
• Experience in adult disability, intellectual disability, rehabilitation, community care, nursing home, or social service settings is preferred.
• Experience in mobility rehabilitation, falls prevention, neurological / musculoskeletal rehabilitation, seating and positioning and functional training is preferred.
• Strong clinical reasoning, communication, interpersonal, and organisational skills.
• Demonstrated ability to work independently and contribute effectively within a multidisciplinary team.
