Rehabilitation Services

All residents receive twice-weekly occupational and physiotherapy sessions, delivered according to individualised therapy plans.

Our Occupational Therapist and Physiotherapist lead a team of eight Therapy Aides to provide both active rehabilitation and active maintenance programmes to help our residents improve and maintain their physical abilities. Each resident is carefully assessed by therapists, and a personalised rehabilitation plan is developed to support agreed goals, independence and quality of life.

Wheelchair Seating and Positioning Support

We regularly review our residents’ wheelchairs and seating to ensure they are safe, comfortable, and well-fitted. Our team makes modifications when needed, and where appropriate, we arrange for new wheelchair prescriptions to better support posture, comfort, and daily activities.

Services

Individual Care Plan

Recognising that each resident has unique needs and aspirations, our Home develops Individual Care Plans (ICPs) tailored to their rehabilitation and healthcare goals. These plans are co-created by our multi-disciplinary team comprising, Nursing Officers, Physiotherapists, Occupational Therapists, and the Social Services team—in collaboration with residents and their families. ICPs are reviewed every six months to ensure they remain relevant and effective. With active participation of our residents and the support of our dedicated care team, these ICPs help our residents in their own journey toward greater independence and quality of life.
