Services

We provide an individual care plan for both our residents and daycare clients.

Our therapists, social workers and nurses work closely together to customise rehabilitation programmes for every beneficiary, so as to maximise their potential for independent living.

Our beneficiaries have individualised occupational and physiotherapy sessions to improve their motor skills. Those with good potential for community reintegration undergo intensive assessment and rehabilitation.

Our beneficiaries are supported by services from our Nurses, Social Workers, Physiotherapist, Occupational Therapist, Psychologist, Speech Therapist and Dietician.

Every beneficiary has a holistic Individual Care Plan which is planned, implemented and reviewed by the multi-disciplinary team with participation from the beneficiary and the family at least once a year.

Our Social Workers provide counselling and emotional support to our beneficiaries and their families. For the more abled residents, they are also supported in their job search by the social worker in collaboration with the therapists.

Our Day Care programme caters to beneficiaries with physical disabilities which require some form of rehabilitation to maintain, enhance and even improve physical and functional capabilities. These programmes are planned by our qualified therapists and assisted by Therapy Aides. Our beneficiaries participate in social interaction through games and other group activities organised by the Home.

The Home provides both long term and short respite care for residents in a family-like environment where residents are also encouraged to undergo regular rehabilitation, engage in recreational and other stimulating activities and take up suitable employment opportunities.

We have four vehicles to provide transport for our beneficiaries to attend their medical appointments, outdoor events/activities, banking, home leave, etc., and ferry Day Care beneficiaries to and from the Home.

Our Home has several structured programmes including hydroponics, hydrotherapy, art and craft, music and movement and other social and recreational activities organised by our regular volunteer groups.

Residential & Respite Care Programme

Our Home provides rehabilitation programmes where residents are offered rehabilitation, stimulating activities, employment opportunities, recreation and encouragement in a home-like environment.

Every effort is made to motivate them within the scope of their capabilities, to live as normal, cheerful, creative, and purposeful lives as independently as they can.

Our residential care service has a capacity to care for up to 90 residents including respite beds. Respite care is offered to caregivers who may need to be hospitalised or take a short break from caregiving.

We typically admit beneficiaries who are young adults and middle aged. Our elderly residents are those who continue to need long-term care in the Home.

A number of our beneficiaries suffer from muscular dystrophy/atrophy, spastic, cerebral palsy, deformities, spinal/head injuries, etc.

Admission Criteria

  • Singapore Citizens or Permanent Residents
  • Age range between 18 and 55
  • Physically disabled but do not require heavy nursing care
  • Free from infectious or contagious disease or any mental impairment or behavioural issue

Day Care Programme

The Home’s rehabilitation programme is designed to enable the physically disabled to achieve their optimum level of independence.

Our Day Care Centre welcomes clients from 9 am to 4 pm, Monday to Friday, except public holidays. We provide lunch and two teas to Day Care clients.

Our Day Care Centre can accept up to 50 clients, offering programmes that are specially catered for every individual to help them achieve an optimal level of independence.

To optimise the rehabilitation of our beneficiaries, we welcome caregivers to attend the Day Care programme so that they can be trained to carry out similar activities with their charges at home.

Transportation

We have four vehicles which we use to fetch our beneficiaries for their medical appointments, homeleave, and to attend our Day Care Centre.

Where seating capacity permits, transport for our Day Care clients along the Home’s designated routes can be arranged.

  • Singapore Citizens or Permanent Residents
  • Age 18 years and above
  • Physically disabled
  • Without mental illness or behavioural problems
  • Mobile, with/without aids
  • Able to feed and toilet with minimal assistance
  • Free from infectious or contagious diseases
